Ambulatory Services
‘Ambulatory care’ refers to care that is provided in community settings, outside of inpatient facilities. Calvary Health Care Bethlehen’s ambulatory services provide neurological and palliative care to patients at home or in residential care facilities, in addition to a range of programs offered through outpatient clinics and day centres.
Our Neurological Ambulatory Services include interdisciplinary outpatient clinics, specialist allied health and neurological nursing consultancy in the community setting, and the Neurological Day Centre.
Our Community Palliative Care Services include an interdisciplinary specialist community-based service, and the Palliative Care Day Centre.
Both Services provide interdisciplinary support to patients at home or in residential facilities by providing practical assistance, care coordination, advice on symptom management, and psychosocial and bereavement support. The community-based and outpatient services are complemented by the day centres, which provide patients with social interaction, promote well-being, and provide day respite for carers.
Both Services also provide consultancy and outreach services to improve patient access to specialist neurological and palliative care across the state. We aim to foster links between local health care professionals and agencies, patients and families, and Calvary Health Care Bethlehem’s interdisciplinary teams. We provide services such as clinical advice, education and training and outreach clinics, including telehealth services for rural and remote locations.
Patients may be referred to Calvary Health Care Bethlehem’s services from another hospital, by their general practitioner, specialist or other health care providers.
