Community Palliative Care Services provides palliative care for people living at home with a life limiting / threatening illness.

The holistic interdisciplinary model of care recognises that the illness affects the whole person and their family / carer(s), physically, emotionally, socially and spiritually.
The practical application of this model includes providing emotional and practical support to both patients and family / carers in the patients home (including aged care facilities), and symptom management including pain relief.
Eligibility for referral to the service includes that the person has a life threatening illness, and resides in the cities of Bayside, Port Phillip, Glen Eira, Stonnington and part of Kingston.
Referrals can be made by the person’s general practitioner, hospital, self or family.

Access to Palliative Care Day Centre and inpatient beds is available for CPCS patients.
The Community Nursing Service (Neurological) of Calvary Health Care Bethlehem was established in 1965. The service delivers home based care to clients suffering from progressive degenerative neurological conditions, such as Multiple Sclerosis, Steele Richardson Syndrome and Motor Neurone Disease.

Allied health is provided by qualified professionals who, through their studies and experience are experts in specific areas of health care. At Calvary Health Care Bethlehem, we offer a broad range of allied health services to
provide holistic care to our patients and their families. Clinicians are available to assist patients
to manage their physical symptoms such as changes in communication, swallowing or mobility and to support
patients and their families with psychosocial issues such as cognitive change, home support, grief and
psychological adjustment. Allied health services are an integral part of the multidisciplinary team who
provide care and support to patients and families of CHCB.
