Geriatric Evaluation and Management

A Geriatric Evaluation and Management Program provides geriatric care,
assessment and management and unplanned
admissions for emergency relief.
A Geriatric Evaluation and Management Program has
four components:
- A management and treatment component for a patient with a
condition requiring acute medical care and management by a
geriatrician and multidisciplinary team. Comorbidities are present
which are expected to lead to an increased length of stay.
- An assessment and management component for an ongoing medical
condition following the acute phase of care, requiring continued
medical intervention and multidisciplinary input to improve level of
functioning. A patient with a medical condition for which an acute
AN-DRG casemix payment has already been made, who requires ongoing
geriatrician and multi-disciplinary management and assessment of
complex medical, psychological, social and/or functional conditions
and needs. These patients require the agency to record a statistical
separation to change Care Type from acute to sub-acute.
- An assessment and management component for reduced level of
functioning without an acute specific medical episode. A patient
with a slowly reducing level of functioning requiring clinical
intervention and management by a geriatrician, in addition to
multidisciplinary assessment or management of complex medical,
psychological, social and/or functional conditions and needs.
- The emergency relief component encompasses unplanned admissions
for care and support of a person in a stable condition requiring
accommodation and clinical care to provide urgent or emergency
relief to carers.
The Geriatric Evaluation and Management
Program excludes Nursing Home Type/ Non-Acute patients.
