Symptom
Assessment & Management
Patients with a chronic
medical condition may at times have difficulty managing
their symptoms. Clinical intervention may be required to
assess and implement a strategy to better control the
symptoms of a condition. A discharge management plan is
initiated for ongoing support as required.
Restoration
CHCB offers a low intensity
rehabilitation program which focuses on optimising the
patient’s wellbeing. This is achieved through improving
strength and endurance and the provision of education
regarding self management strategies. An
interdisciplinary approach involving a defined
management and discharge plan is implemented with the
patient’s needs central to the treatment plan. Patients
who may find this beneficial are those who have become
deconditioned as a result of illness or injury
End of Life
Care
Patients with advanced
illness require a range of services to ensure their
physical, psychological, social and spiritual needs are
met effectively and to enable them to live and die in
the place of their choice, if possible.
