Inpatient
Services
An
inpatient admission to CHCB may be required for patients
who are unable to be treated in the community. Of our 70
beds we have 32 Palliative beds, 30 progressive
neurological beds and 8 geriatric medical beds.
Patients may be referred to Calvary Health Care
Bethlehem from another hospital, by their general
practitioner or specialist. The community service also
accepts referrals from patients, their families and
carers.
To
access our inpatient beds, patients have to be known to
our community palliative care service or have been
assessed by a specialist palliative care provider. The
palliative care service works in close consultation with
the patient's GP and specialists to provide safe and
compassionate care.
All
patients are assessed to determine individual needs and
cared for by an interdisciplinary team. Where
appropriate therapy may be provided in the ward
environment or in the therapy departments.
Coordinated discharge planning provides a team approach
to address the requirements of the patient following
their inpatient stay.

Outpatient
Services
Through the assessment
clinic, people can access the multi-disciplinary
services of the Neurology Unit team on an outpatient
basis.
The clinic is lead by the
Clinical Director of Neurology, who provides
neurological assessment and management, and coordinates
referrals to appropriate allied health, nursing and
social support services within the neurology unit.

Community
Services
Our specialised community
team support patients with a progressive condition by
providing practical assistance, a range of therapies,
advice on symptom management, emotional support as well
as bereavement support to families.
The service delivers home
based care to clients suffering from malignant and non
malignant life threatening conditions such as Cancer,
Cardiac Failure, Airways Disease or Kidney Failure.
Patients with progressive degenerative neurological
conditions including Multiple Sclerosis, Huntington’s
Disease, Parkinson’s Disease and Motor Neurone Disease
may also benefit from this home based service.
An interdisciplinary team of
health professionals is available in the community
offering a broad range of services Apart from nursing
and medical management clinicians are available to
assist patients to manage their physical symptoms such
as changes in communication, swallowing or mobility.
They also support patients and their families with
psychosocial issues such as cognitive change, home
support, grief and psychological adjustment.

Day Centre
CHCB provides a range of services which help
people adjust to the physical, social and psychological
limitations of their illness. Patients who may be
suffering from a malignant or non malignant condition
will often benefit from the scope of activities offered
through the day centre.
The
interdisciplinary team includes: diversional therapy,
medical/nursing, music therapy, pastoral care and many
more. The services provided include: social and creative
activities, respite break, gentle exercise programs,
relaxation techniques etc.
The
aim of the centre is to assist well-being and optimise
quality of life, for patients and their family/carers in
a comfortable, homely and secure environment.
The
Day Centre is available to both inpatients and
outpatients and the hours of operation are; 9.00 am to
3.00 pm, Monday & Wednesday (excluding public
holidays).
